Service System Coordination
Service System Coordination is a MCYS-funded service available in those cases where, because of the number of services and supports required and/or the complexity of the situation, additional service coordination is required. This service is and independent service and offered in collaboration with Children's Centre Thunder Bay, Dilico Anishinabek Family, Integrated and North of Superior Counselling Programs.
Who Can Access This Service?
Residents of Thunder Bay and District, under the age of 18 with ongoing functional limitation/s as a result of a disability and/or disorder as documented by a physician, psychologist, or psychiatrist; and have support needs which are beyond the available services and supports offered within Thunder Bay and District.
Inclusionary Criteria
- A diagnosis of more than one disability-related need which can include the following: Developmental Disability (I.Q. below 70), Mental Health diagnosis, Physical Disability, Autism Spectrum Disorder, Medically Fragile Technologically Dependent, Acquired Brian Injury or Rare/Unique/Extreme Condition.
- The diagnosis and accompanying difficulties must transcend a number of environments and involve multiple services.
- Documented history of exhausting all available resources, therefore demonstrating that the referred individual is "most in need".
Exclusionary Criteria
- The complexity of service needs for the child/youth are not beyond the capacity of the service system and family to address.
- Failure to demonstrate that all available resources have been exhausted.
What Is Offered?
A collaborative process facilitating the removal of service barriers in order to create, design or develop a clinically supported individual service plan which meets the multiple complex needs of the referred client.
How Will This Service Help?
As a representative of the Service System, the service system will work in partnership with you to create a plan which addresses your complex/multiple needs. Some benefits of involvement with this program are coordinated services, a decreased risk to the immediate heath and safety of the child, preservation of the family unit, and someone to advocate for your needs.
How Can You Find Out More Or Access This Service?
Referrals for Service System Coordination tend to be made by professionals such as social workers on behalf of children and families. However, referrals for service may also be accepted from parents, guardians and youth.
Enquiries can be made by contacting the Children's Centre at 343-5000 and asking to speak to the Supervisor, Service System Coordination. This staff will work with you to determine whether or not a referral to this service is appropriate.
