Making a Third Party Referral
Third Party Referrals Welcome
Third Party referrals are accepted for Counselling and Therapy. Completed referrals can be sent through the online referral form below, or by completing the printable referral form.
Referrals for our Live-In Treatment Program, Preschool Speech and Language, Blind/Low Vision, and Infant Hearing Program are completed by telephone only.
Referrals can also be made by phone to Intake at 1-855-486-5037 or 807-343-5046. Please note that summer hours for Children's Centre Thunder Bay are in effect from the end of June until the first week of September.
Printable referral forms are available by opening the referral form and right clicking to reveal a pop-up window where the option to print will appear and can be chosen. Printed and completed referral forms can be faxed to 807-345-0444, or mailed to:

Incomplete referral forms will not be accepted.
To complete a referral, the following information must be included:
- Client consent, name and contact information
- Permission to call
- Preferred language and ethnicity
- Next of kin and custody
- Reason for referral and presenting issues
- Risk factors
- Information about the referring party
Once a referral is submitted, you will receive a link that will allow you to check the status of your referral. Please note that submitting a referral does not mean your client/patient has been added to the wait list
By proceeding with the completion of this referral form, you are acknowledging that your client/patient has provided you with consent to complete this referral on their behalf.
